Welcome to Silver Oak Health Employee Wellbeing and Assistance Program (EWAP) and we appreciate the confidence you have placed in us.

Through this e-consent you are participating voluntarily in counselling services from a mental health counsellor and that you have the option to withdraw from this service any time. Further you acknowledge and agree that in order to provide the counseling services under the EWAP, we may directly collect your "Personal Information," that specifically identifies you as an individual, such as your full name, telephone number, e-mail address, postal address, gender, age, year of birth, and employee id.

By giving your consent, you understand that the Counsellor will be providing certain therapy and/or counselling services, and acknowledge that numerous factors can affect the overall outcome and that such therapy and/or counselling services may not be effective in accomplishing the goals or other desired outcomes as discussed. You acknowledge and agree that there may be circumstances that presently, or in the future, require treatment or other health care services that are outside the scope of the services that can be provided by this counsellor. With full knowledge of the risks and benefits of engaging in therapy and/or counselling services, I give my informed consent by clicking the "LOGIN".

Limits of Confidentiality:

With few exceptions (listed below) what you disclose in the therapy relationship is confidential and will not be disclosed to anyone outside of the session without your permission.

If Silver Oak Health Counsellors have reason to believe any of the following are true, our counsellors are obligated by law to take certain actions to protect your health and safety (and that of others) and to remain compliant with local mental health laws. Exceptions to confidentiality include:
  • If our counsellors believe that you are in danger of causing harm to yourself
  • If our counsellors believe that you are in danger of harming another person
  • Our counsellors are required to report any situation of suspected child abuse

The above list is not exhaustive as other instances may arise on an individual basis that require discussions of confidentiality and disclosure. In most cases the above situations rarely occur, or if they do occur, will be discussed directly with the client who will be fully aware of disclosures of information and associated interventions. Our counsellors will make every effort to engage the client in discussions about safety of self and others throughout the course of therapy and attempt to engage the client in active participation in safety plans/interventions should these instances arise.
